SMS-Text Message Terms

1. Faith Medical Services SMS Program

Faith may use text messaging to communicate with individuals who have an existing or prospective relationship with Faith, including:

• Clients
• Authorized family members or representatives
• Prospective clients
• Employment applicants
• Employees
• Field employees
• Referral sources
• Healthcare professionals
• Other authorized business contacts

Messages may be sent by authorized Faith personnel using Faith's business telephone and messaging systems.

2. Types of Messages

Depending on the recipient and their relationship with Faith, text messages may include:

• Appointment confirmations
• Appointment reminders
• Scheduling communications
• Service updates
• Referral updates
• Client or caregiver communications
• Responses to service inquiries
• Employment application updates
• Recruiting communications
• Employee communications
• Billing or payment-related communications
• Customer-service communications
• Responses to messages initiated by the recipient
• Information concerning Faith events or activities
• Other communications directly related to Faith's services or business relationship

Faith does not intend to use this SMS program for general advertising or promotional marketing campaigns.

If Faith introduces a separate marketing or promotional SMS program in the future, Faith will obtain any consent required for that program and will provide the disclosures applicable to that program.

3. Consent

Where required by applicable law or messaging-provider requirements, Faith will obtain appropriate consent before sending text messages.

Consent may be provided through an appropriate website form, written communication, electronic communication, verbal communication where legally permitted, or another documented method appropriate to the type of message.

Providing a telephone number does not automatically constitute consent to receive every type of text message from Faith.

Consent to receive one category of message does not necessarily constitute consent to receive unrelated marketing or promotional messages.

Where a separate SMS consent checkbox or other affirmative consent mechanism is provided, it will be separate from acceptance of Faith's Terms of Use and Privacy Policy.

SMS consent is voluntary where separate consent is requested, and declining SMS communications will not by itself prevent a person from requesting services, applying for employment, or otherwise interacting with Faith, except where a particular communication method is independently necessary and permitted by applicable law.

Message frequency varies.

The number of messages you receive will depend on your relationship with Faith, the services or employment activities involved, appointments or scheduling needs, and the communications you initiate or authorize.

Message and data rates may apply.

Your mobile carrier's standard messaging and data rates may apply to text messages sent to or received from Faith.

Faith does not control the rates charged by your mobile carrier.

You may request that Faith stop sending text messages by replying STOP

When supported by the applicable messaging system, replying STOP will be treated as a request to opt out of future SMS communications from the applicable Faith messaging program.

Please understand that opting out of SMS messages does not necessarily end other forms of communication. Faith may continue to communicate by telephone, email, mail, or another permitted method when necessary to provide services, process an application, administer employment matters, or otherwise communicate with you.

You may also receive a limited administrative message confirming your opt-out or otherwise necessary to process your request.

8. Healthcare and Confidential Information

Faith provides healthcare-related services and may receive protected health information.

Faith will take appropriate precautions concerning confidential and protected health information.

Please do not send highly sensitive medical information through ordinary text messaging unless Faith has specifically instructed you to use that communication method and the communication is appropriate under applicable law and Faith's policies.

When appropriate, Faith may use email, secure electronic systems, healthcare platforms, telephone communication, or other methods instead of ordinary SMS.

Faith will not intentionally use ordinary SMS as a substitute for an appropriately secured healthcare communication system when a more secure method is required.

9. No Sale or Marketing Sharing of Mobile Information

Faith does not sell or rent mobile telephone numbers or SMS consent information. Faith will not share mobile telephone numbers, SMS opt-in information, or messaging consent with third parties or affiliates for their own marketing or promotional purposes.

Faith may use communications and technology service providers to transmit or support messages on Faith's behalf. Such providers are not authorized to use Faith's SMS consent information for their own unrelated marketing purposes.

10. Message Delivery

Faith does not guarantee that every text message will be delivered immediately or at all.

Message delivery may be affected by mobile carriers, network conditions, device settings, geographic location, technical problems, outages, or other circumstances outside Faith's control.

Carriers are not liable for delayed or undelivered messages.

11. Changing Your Telephone Number

If you change or discontinue the mobile telephone number associated with your communications with Faith, please notify Faith so that we can update our records.

You should not allow another person to use a telephone number that you previously provided to Faith without notifying us if you no longer control that number.

12. Separate Consent for Marketing

Faith does not currently intend to use this SMS program for general promotional marketing.

If Faith later creates a separate promotional or marketing text-message program, the applicable consent and disclosures will be provided before enrollment where required.

Consent to receive service-related communications does not automatically constitute consent to receive unrelated promotional messages.
