Faith Medical Services provides personal support services with day to day tasks to individuals in Atlanta, Georgia. Personal Support Services aim to assist disabled individuals or people with a chronic health condition by helping them live an independent life in their own homes; rather than being placed in a hospital or nursing facility. This type of service provides patients with the opportunity to continue to be part of their community and live close to their loved ones despite their condition.
What are Personal Support Services?
The Personal Support Services program focuses on delivering all day-to-day services for your loved ones who can no longer care for themselves due to a chronic health condition or a disability.
Personal Support Services provide assistance from a skilled caregiver with the most basic tasks that for your sick loved one seem challenging, including:
- Help with personal hygiene
- Bathing and showering
- Haircare
- Brushing teeth
- Dressing and changing clothes
- Medication reminders
- Help with moving around the home
- Help with transferring from a bed, chair, or other structure
- Help with moving, turning, and positioning the body while in bed or in a wheelchair
- Light housework (help with laundry, dusting, vacuuming, trips to doctor appointments or outside activities, shopping, meal preparation, etc.)
If one of your loved ones finds it challenging to manage all these tasks on their own due to a medical condition, our licensed and skilled caregivers will provide them with assistance based on their individual needs.
Am I eligible to receive Personal Support Services?
The Personal Support Services program aims to help those who are medically unable to perform daily tasks such as eating, dressing, bathing, toileting, or mobility on their own.
To qualify for the Medicaid program, applicants must have a chronic medical condition, a disability, or cognitive impairment. The applicants must also demonstrate they need support with three of the five activities of daily life (ADLs), two ADLs, one of which requires assistance at a fell dependence level, or two ADLs, one of which involves support at a full dependence level.
