1. Your Rights

You have certain rights regarding your protected health information.

Request a Copy of Your Health Information

You generally have the right to inspect and obtain a copy of protected health information maintained by Faith, subject to applicable legal limitations.

Request a Correction

You may request that Faith correct protected health information that you believe is incorrect or incomplete, subject to applicable legal requirements.

Request Restrictions

You may ask Faith to restrict certain uses or disclosures of your protected health information. Faith is not required to agree to every requested restriction except where required by applicable law.

Request Confidential Communications

You may request that Faith communicate with you about health matters by a particular method or at a particular location. Faith will consider reasonable requests as required by law.

Obtain a Paper Copy

You may request a paper copy of this Notice even if you have agreed to receive the Notice electronically.

File a Complaint

You may complain to Faith if you believe your privacy rights have been violated. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Faith will not retaliate against you for filing a privacy complaint.

2. How Faith May Use or Disclose Your Health Information

Faith may use or disclose protected health information as permitted or required by applicable law.

Treatment

Faith may use or disclose health information to provide, coordinate, or facilitate healthcare and related services.

Payment

Faith may use or disclose health information as necessary to obtain payment for services.

Healthcare Operations

Faith may use or disclose health information for healthcare operations, including activities necessary to operate and manage our services, subject to applicable law.

Individuals Involved in Your Care

Faith may disclose appropriate information to a family member, caregiver, personal representative, or other person involved in your care when permitted by law and appropriate to the circumstances.

Required by Law

Faith may use or disclose health information when required by federal, state, or local law.

Public Health Activities

Faith may disclose health information for legally authorized public health activities.

Health Oversight

Faith may disclose health information to governmental agencies for authorized oversight activities.

Judicial and Administrative Proceedings

Faith may disclose health information in response to lawful proceedings and legal requirements.

Law Enforcement

Faith may disclose health information for certain law-enforcement purposes when permitted or required by law.

Serious Threats

Faith may disclose health information when necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public, when permitted by law.

Workers' Compensation

Faith may disclose health information as necessary to comply with workers' compensation laws.

Other Uses and Disclosures

Other uses and disclosures of protected health information may require your written authorization. If you provide authorization, you may generally revoke that authorization in writing to the extent permitted by law.

3. Faith's Responsibilities

Faith is required by law to:

• Maintain the privacy of protected health information
• Provide individuals with this Notice describing our legal duties and privacy practices
• Follow the terms of the Notice currently in effect
• Notify affected individuals when required by law following a breach of unsecured protected health information

Faith reserves the right to change its privacy practices and this Notice. If Faith makes a material change to this Notice, the updated Notice will be made available as required by law.

4. Confidentiality of Client Records

Faith maintains client records containing confidential information.

Faith uses administrative, physical, and technical safeguards designed to protect client information from unauthorized access, use, disclosure, alteration, or destruction.

Access to client information is limited to authorized individuals who have a legitimate need for the information and are permitted to access it under applicable law and Faith policies.

Faith personnel are required to maintain confidentiality of client information.

5. Disclosure With Authorization

Except as permitted or required by applicable law, Faith will obtain appropriate authorization before making uses or disclosures of protected health information that require authorization.

When a disclosure is made pursuant to an authorization, the authorization and any applicable restrictions will be handled according to applicable law.

6. Text Messages and Electronic Communications

Faith may use telephone, email, text messaging, healthcare systems, or other communication methods to communicate with clients and authorized individuals.

Because ordinary SMS and email may not provide the same protections as an appropriately secured healthcare system, Faith may limit the amount or type of health information communicated through those methods.

Individuals should avoid sending highly sensitive medical information through ordinary text messages or general website forms unless Faith specifically instructs them to do so.

7. Record Retention

Faith maintains client records according to applicable law, regulatory requirements, contractual requirements, and Faith's records-retention policies.

Faith's internal records procedures may provide for retention periods of at least five years from the date of last service where applicable. Certain records may be retained longer when required or appropriate.

9. Acknowledgment

Where required, Faith will request an acknowledgment that an individual received this Notice of Privacy Practices.

An acknowledgment of receipt does not mean that the individual agrees to or authorizes uses or disclosures of protected health information beyond those permitted by law.
